• 2 Attending physicians (8:30 am - 4:00 pm)
• 1 Nurse Practitioner (8:30 am - 4:00 pm)
• 1 Dietitian, afternoon only (1:00 pm- 4:00 pm)

• 1 Nurse Practitioner (8:30 am - 4:00 pm)

TUESDAY:

1400-1430 Sweet Home Road, Amherst

• 1 Nurse Practitioner (8:30 am - 4:00 pm)

WEDNESDAY:

140 Hodge Avenue, Women & Children’s Hospital

• 1 Nurse Practitioner (8:30 am - 4:00pm)
• 1 Attending physician (1:00 pm - 4:00 pm)
• 1 Dietician (8:30 am - 4:00 pm)

How to make an initial appointment:

When making a new patient appointment, we ask that you contact your primary care physician first. Your primary care physician will then call our office and set up an appointment for the first available time. It is in the best interest of your child that we have the full details of your primary physician’s concerns.

Because there are several Health Insurance Carriers, it is also important that your primary care physician is aware of your visit with us in case a referral is needed. We ask that any testing and/or X-rays that may have been done through your primary physician’s office also be forwarded to us; this will reduce the amount of repeat testing. Once the initial appointment has been made, we will be glad to assist you with any schedule changes.

Women & Children’s 140 Hodge Avenue Outpatient Clinic: (See the map)

From the North: Take 190 South to the 198 (Scajaquada) and exit at Elmwood Ave. Take Elmwood to Hodge Avenue and turn left on to Hodge.

Coming from the South: Take the 190 to Porter Ave. (Peace Bridge) Exit. Turn left on Porter Ave. Take Porter to Richmond, and turn left on to Richmond. Take Richmond to West Utica and turn right onto West Utica. Take West Utica to Elmwood. Turn right on Elmwood to Hodge, and make a left onto Hodge Avenue.

From the West (Canada): Take the Peace Bridge; bear to the left and take the exit to Porter Ave. Turn left on Porter Ave., and take Porter to Richmond. Turn left onto Richmond. Take Richmond to West Utica, make a right turn on West Utica. Take West Utica to Elmwood, turn right on Elmwood. Take Elmwood to Hodge, turn left onto Hodge.

Amherst Center - 1400-1430 Sweet Home Road Suite 5 (See the map)

From the North: Take Rt. 990 to Sweet Home Road exit 2a. Follow Sweet Home Road past Sweet Home High School. Turn right at the signal in front of the entrance to the University of Buffalo, that will be Rensch Rd. make a right into the parking lot.
